This form can be used to request a change both your account holder and data contact for DataManager, Riverside Score, or 

Before You Begin Please Note:

• If you're a Canadian user, we ask that you contact Nelson Educators for assistance.
• Data stored in Riverside Insights' scoring systems is sensitive and private information.
  • We need to ensure that any changes to the account are designed to preserve the integrity and privacy of that data.
• This form can also be used to request a change to your Data Contact.
  • The DataManager platform services our Iowa Assessments™, Cognitive Abilities Test™ (CogAT®), and Logramos® assessments.

• Once this request is processed, the current Account Holder will lose all access.
• It may take up to 5 days for your request to be fulfilled.
